We welcome your feedback, concerns and complaints about our services. It helps us understand what’s working well and where we need to do better, and it supports safer, more respectful care for everyone we work with.
You can submit this form anonymously. If you choose the anonymous option, we won’t ask for your name or contact details. Making a complaint will not affect your care or supports, and we will treat all feedback confidentially and respectfully.
If you would like a response, please include your preferred contact details and tell us the best way to reach you. We will acknowledge your message and keep you informed of the outcome where possible. If you submit anonymously, we may be limited in how we can clarify details or update you, but we will still review what you tell us and act where we can.
If anyone is at immediate risk or you need urgent help, call 000. If your concern relates to a serious incident, please contact us as soon as possible so we can respond quickly and put supports in place.
You can also contact external bodies at any time. For aged care concerns, you may contact the Aged Care Quality and Safety Commission. For NDIS-related concerns, you may contact the NDIS Quality and Safeguards Commission.
