Did I automatically transition to Support at Home?

Yes. If you previously received a Home Care Package, you’ll be supported to transition into the new Support at Home program from 1 November 2025. You won’t need to reapply.


Will my current services continue?

In most cases, yes. Reliant will work with you to ensure continuity of care, though some services may be delivered differently under the new funding model. We’ll keep you informed every step of the way.


Do I need to do anything now?

Not right away. However, we recommend speaking with our team to prepare and understand what’s changing. We’ll help you feel confident and ready.


Will there be changes to how I pay for care?

Yes, the government is introducing new funding and payment arrangements under Support at Home. We’ll explain any changes clearly, including how it may affect your fees and contributions.


Will I get more say in my care?

Absolutely. One of the key goals of the new program is to give you more flexibility and choice in how your services are delivered. At Reliant, that’s how we’ve always operated.

What is Support at Home?

It’s a new Australian Government program starting on 1 November 2025 that replaces Home Care Packages. It aims to make in-home care simpler, more flexible, and easier to access.


How do I know if I’m eligible?

You’ll need to register with My Aged Care and undergo an assessment by the Aged Care Assessment Team (ACAT). We can guide you through the process step by step.


When should I start applying?

If you or a loved one may need care in the next 6–12 months, we recommend starting now. Waiting lists can be long, and getting assessed early means you’ll be ready when Support at Home begins.


What kind of help can I get?

Support at Home covers services like personal care, nursing, help around the house, social support, allied health, and more. Our team will help design a plan tailored to your needs and lifestyle.

Compare Support at Home funding levels

Every Support at Home classification comes with a yearly budget from the Australian Government. Ten per cent of that budget is set aside for care management, and the rest can be used for services like personal care, domestic help, shopping and social support.

Because Reliant uses one simple hourly rate for most care-worker services, it’s easier to see what your funding can translate to in approximate hours of support each week.

These examples are a guide only. Actual hours will depend on when you have visits (weekdays vs weekends and public holidays), whether you use nursing or allied health, and any other costs in your plan.
