When does 24-hour care at home make sense?

Most families do not suddenly decide they need round-the-clock care. Usually the need builds: more help overnight, another fall, a hospital admission, increasing confusion, or a family carer who is simply running out of capacity.

Someone cannot safely be left alone

Long periods alone are becoming unsafe because of mobility, memory, confusion or changing health needs.

Help is needed overnight

Bathroom assistance, reassurance, repositioning or other support is regularly needed after hours.

Family care is no longer sustainable

A partner or family member is carrying too much of the day-to-day and overnight care.

Needs changed after hospital

A discharge has changed what can safely and realistically be managed at home.

Care needs are becoming complex

Personal care, mobility, medication routines or progressive conditions now require more coordinated support.

Home remains the preferred option

The person wants to stay in familiar surroundings rather than move into residential care.

What does 24-hour home care actually look like?

It does not mean one person working a 24-hour shift. A planned team provides support across the day and night, with the arrangement matched to the person's actual needs.

1

Daytime support

Personal care, meals, mobility, appointments, companionship and help around the home.

2

Sleep-over care

A carer remains in the home overnight and is available if assistance is needed.

3

Active overnight care

A carer remains awake through the night when regular assistance or monitoring is required.

4

Full 24-hour care

A small team rotates across day and night so support is consistently available.

Care does not have to begin at the highest level. Many arrangements start smaller and increase as needs change.

Care built around one person — not simply a roster

For someone receiving substantial support at home, continuity matters.

Reliant aims to build a small, familiar care team around each client wherever possible.

That means carers get to know the person's routines, preferences and what is normal for them.

That familiarity also makes it easier to notice when something has changed.

Behind the day-to-day care sits clinical oversight and a care plan that can be reviewed as needs change.

What families should expect

  • A clear care plan
  • A familiar team wherever possible
  • Clinical oversight when needed
  • Clear communication with family
  • Coordination with doctors and other health professionals where appropriate

Coming home from hospital?

Hospital discharge is one of the most common times families realise the previous care arrangement is no longer enough.

Someone may be medically ready to leave hospital but still need significant help with mobility, showering, dressing, meals, toileting, medication routines, overnight assistance or simply being safe when nobody else is there.

Private care can be arranged directly while longer-term funding or support arrangements are still being organised.

If discharge is happening quickly, you can also read about Rapid Response Care after hospitalisation or a health event.

Talk to Reliant

Before discharge, ask:

  • Can the person safely be alone?
  • Will help be needed overnight?
  • Who will assist with personal care?
  • Who is managing medications and follow-up appointments?
  • Can the family realistically provide the amount of support required?

Private care gives families flexibility

Private care is arranged directly with Reliant. There is no requirement to wait for government funding before care can begin.

Start when you need it

Care can often be organised more quickly than waiting for a funded pathway to be established.

Choose the level of support

Care can be increased or reduced as the person's needs change.

Use it short or long term

For some families it bridges a recovery period. For others it becomes the preferred long-term arrangement.

Clinical oversight when needs are complex

Twenty-four-hour care often involves more than assistance with everyday activities.

People receiving this level of support may also have complex health conditions, significant mobility limitations, progressive neurological conditions or changing clinical needs.

Reliant Healthcare is RN-led. Registered nurses are part of the clinical oversight of care rather than being completely removed from the day-to-day service.

Where appropriate, the team can coordinate with GPs, specialists and other health professionals involved in the person's care.

The aim is not to make home feel clinical.

It is to have the right clinical judgement available when it matters.

Health professionals can make a referral online.

Frequently asked questions

How much does 24-hour home care cost?

The cost depends on the number and type of shifts required, whether overnight care is active or sleep-over care, and the complexity of support. Reliant will explain likely costs before care is agreed.

Does someone stay awake all night?

Not necessarily. Some people need a sleep-over carer who remains in the home and can assist if required. Others need active overnight care, where the carer remains awake because regular support is required.

Can we start with less than 24-hour care?

Yes. Care can start with additional daytime support, overnight care or another arrangement and increase if needs change.

Can care start after someone leaves hospital?

Yes. Hospital discharge is a common reason families arrange private care. Where possible, speaking with us before discharge gives everyone more time to understand what support will be needed at home.

How quickly can care be arranged?

Timing depends on the level of care required, location and staffing needs. If care is needed quickly, call us directly and we will tell you what is realistically possible.

Does Reliant provide nursing as part of 24-hour care?

Where clinical care is required, Reliant's registered nurses can be involved in assessment, clinical oversight and nursing services. Not every 24-hour care arrangement requires a nurse physically present around the clock.

You do not need to know exactly how much care is needed before you call.
